24.13. High Risk Waiver Setup
High risk waivers allow you to inform owners of increased surgical or treatment risk and collect a digital signature. You can also perform verbal high risk waivers if getting a signature is not possible. Each type of high risk waiver requires its own setup.

To use these tools, your clinic must be using Patient Flow.
Verbal High Risk Waiver Setup
- Go to Settings > High Risk Waiver Setup.
- Click Load Default Template, and edit template to suit your clinic and click Save Changes.
- If you'd like HQ to require the "Witness Name" field be filled out before being able to process a High Risk Waiver, tick the setting Witness Is Required. The HRW Reasons button is a placeholder field that will fill in with the reason for HRW selected on your exam. #_ Go to Settings > Medical Notes > High Risk Waivers Triggered Notes section.
- Review the default notes we have included in your HQ setup. If you need to edit a note, click the gear icon then click Edit. You can also delete a note if it will not apply to your clinic.
- For reasons without pre-populated notes, click Add Item and select the High Risk Waiver item. Enter the description in the External Client Communication textbox. This will populate the Script for the verbal High Risk Waiver. To better understand how this part works, click here.
Email High Risk Waiver Setup
This only difference here is you will set up a template tagged as your clinic's High Risk Waiver. To set this method up, follow these instructions.
- Go to Settings > Templates and upload your clinic's High Risk Waiver as a new template. Be sure to tag it as a "High Risk Waiver." Note: You can only have one High Risk Waiver template.
- We recommend using this one: You can download it, change the logo and other clinic details, and add or subtract anything you like.
