9. Understanding Various Paperwork (Templates)

Paperwork Makes the World Go Round!
Especially for spay/neuter clinics, paperwork is essential for organizing daily operations and ensuring proper documentation. This section covers the most common types of paperwork used in spay/neuter and wellness clinics.
- All templates can be downloaded from HQ University.
Common Paperwork Types
- Consent Forms ** Provide owner permission for procedures such as sterilization or treatments. ** Outline the procedure and its risks. ** Signed by the owner to confirm understanding and release the clinic from liability.
- Treatment Sheets ** Record physical exams, anesthesia details, services provided, rabies vaccine information, and additional notes.
- Cage Cards ** Printable cards to place on cages for easy patient identification.
- Take-Home Paperwork ** Aftercare instructions, rabies certificate, and a summary of the appointment.
- Medical Record ** The official patient record. ** Includes appointment summaries, treatment sheets for each visit, and any diagnostic results (e.g., Idexx).
- High-Risk Waivers ** Signed by clients for patients at increased surgical or treatment risk.
- Rabies Certificate ** Included in HQ by default, though it is recommended to integrate this into your Take-Home Paperwork for efficiency.
- Spay/Neuter Certificate ** Optional, as most clinics use Take-Home Paperwork in place of a separate certificate.
- Roster ** A daily summary of appointments. ** Used by clinics to organize and prepare for the day.
